とある建物の一室で目を覚ました12人の男達。彼らは記憶を無くしており、自分が誰かも曖昧な状態だった。 そこに「サラ」と名乗る謎の女性が現れ、ここが「ミラーワールド」という鏡の中の異世界であること、出る為にはライダーとなって1週間以内に最後の1人になるまで殺し合わなければならないことを告げる。 突如として極限状況に置かれ困惑する彼等。しかし、その手にはライダーの力そのものであり、彼らが「仮面ライダー」であったことを表すカードデッキが握られていた。 一方その頃、現実世界では一人の男が警官隊と対峙していた。 男はアナザーウォッチを使ってアナザーライダーに変身し人々を襲撃し続けており、ソウゴやゲイツも対抗する術がなく手をこまねていた。 また、どこかの病院のベッドには、サラが寝たきりになっており……？
